My point is that someone has to be the specific voice of the act/process, or the risk of errors and problems being overlooked or swept under the carpet is increased.
When doing improvement projects, using recording tools like the A3, there is always someone who is accountable for the actions and schedules, and recording outcomes to ensure they are either implemented or reviewed and learnt from.
In my experience, not having a name on a sheet means nobody is accountable, although, by my definitions, all those who touch the project carry responsibility for performance…

Without both parties making themselves accountable for following through, a commitment will only ever be an agreement
